Identifies problems and establishes measurable goals Coordinates the plan of care with members of the care provider team and other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive plan of care Who you are: Current, active LPN License in the State of Arizona by the Board of Registered Nursing. Current CPR, health clearance and clinical skills self-assessment. Minimum of two years of professional nursing experience. Minimum of two years of experience as a clinical supervisor or charge nurse in a hospital or home care setting. Full job description will be provided upon request
