Team Select is a leading provider of in-home healthcare services, dedicated to delivering compassionate and personalized care to our clients.We are seeking a dedicated and compassionate Home Health Private Duty Nurse (RN) or (LPN) to join our team. The ideal candidate will have a strong clinical background and *MUST HAVE TRACH/VENT EXPERIENCE!*****This role requires a nurse who can meet the medical needs of the patient. The ideal candidate must have experience in trach and ventilator care management. Strong attention to detail and compassionate care are essential.*****Schedule: *Two to Three overnight shifts10 PM - 6 AM or 11 PM -7 AM*Key Responsibilities:** Provide high-quality care.* Pediatric Care Experience* Monitor and assess patients' health conditions, including vital signs, and report any changes to the supervising physician.* Administer medications, treatments, and other prescribed interventions.* Trach/Vent management* Maintain accurate and timely documentation of patient care and progress.* Collaborate with the healthcare team to develop and implement individualized care plans.*Qualifications:** Valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Arizona.* Current CPR Certification* *Must be able to work with Trach and Vent in home health setting** Strong Clinical Skills* Reliable transportation and willingness to travel to patient home within the designated area.*Pay Rates:*LPN $28-$32 hourlyRN $38-$40 hourly#PHXLPNJob Types: Full-time, Part-time, PRNPay: $35.00 - $39.00 per hourBenefits:* 401(k)* Dental insurance* Health insurance* Vision insuranceMedical Specialty:* Home HealthPhysical Setting:* Long term careSupplemental Pay:* Signing bonusExperience:* Trach: 2 years (Required)* Ventilator: 2 years (Required)Ability to Commute:* Avondale, AZ 85392 (Required)Ability to Relocate:* Avondale, AZ 85392: Relocate before starting work (Required)Work Location: In personby Jobble
