occupational medicine health services and electronic health records
management for sworn staff from both the City of Phoenix (City)
Fire and Police Departments as well as public safety personnel
assigned to other jurisdictions throughout the region who have
contracted with Phoenix Fire Department Health and Wellness Center
(PFDHWC) for these services. Valor is different because of our

to none. Location Phoenix Fire Department Health and Wellness
Center (PFDHWC) 50 S. 12th St., Phoenix, AZ 85034 PFDHWC Hours
Facility hours are 7:30 a.m. to 4:00 p.m. Monday through Friday.
PFDHWC Clinic hours are 7:30 a.m. to 12:00 p.m. and 12:30 p.m. to
4:00 p.m. Monday through Friday, with a 30-minute lunch period.
Holiday Closures The PFDHWC is closed on all City of Phoenix
observed holidays. Requirements: Responsibilities: Ensure all
required forms and provider notes are sent to City’s Human
Resources Safety Division (HR Safety) for claim processing within 2
business days of patient visit date. Obtain authorization for
referrals for diagnostic testing, physical therapy, and specialists
from either HR Safety or the Third-Party Administrator for
Firefighter treatment. Coordinate the location of treatment with
patients, providers, and involve Patient Liaison as necessary. Act
as liaison between patients and City’s Third-Party Administrator.
Respond to inquiries both verbal and in writing from Third Party
Administrator and HR Safety. Provide Third-Party Administrator with
daily patient notes. Manage the PFDHWC Daily roll call and
Telestaff to ensure the members’ work status is correct for
Emergency Response Staffing Office (ERSO). Follow PFD protocol for
mandatory follow-up appointments. PFD Management Volume I
Procedures 103.02E, 103.06, and 104.02B until the injured worker
has reached Medical Maximum Improvement (MMI) or is cleared for
full duty by the specialist and the PFDHWC contracted provider.
Schedule, check-in, and out-patient, and reschedule appointments.
Respond to patient inquiries via email and phone call within same
business day. Collaborate with healthcare teams, internally as well
as externally (specialists, therapists, etc.). May cross cover
PFDHWC’s administrative staff with transitional work assignments.
Cross covers the Front Office-Scheduler and Medical Records.
Minimum Qualifications: Five years of case coordinating or working
in a medical front office. Knowledge of Industrial Commission of
Arizona forms and claims filing process. Worker’s Compensation case
management experience. Excellent verbal and in writing
communication skills. Must be able to use various personal computer
software programs including, but not limited to, electronic medical
records system, Outlook, and Microsoft Suite. Detail oriented with
the capability to work under stressful conditions and meet
deadlines. Ability to prioritize and handle a variety of tasks
simultaneously, with frequent interruptions. Preferred
Qualifications: Three years of experience working in an
Occupational Health Clinic treating first responders. Working
knowledge of the NFPA 1582 Standard on Comprehensive Occupational
Medical Program. This job is CONTINGENT upon contract award, client
approval, completion of a favorable background investigation, and
the ability to obtain and maintain a Public Trust / High Risk
clearance. Final compensation will be based on relevant factors
including work experience, skills, certifications, and internal
