work.---The Senior Clinical Administrative Coordinator--is a
pivotal member of the Landmark Interdisciplinary Team. As a Care
Coordinator Lead, you will use your clinical skills and
patient-first mentality to assist patients with care coordination
needs, conduct telephonic surveys to collect clinical data, and
support patients in scheduling home visits. The Care Coordinator
Lead is also responsible for the team's performance in customer
service, schedule management, patient visit reminder calls, and
management of consents and medical records, disenrollment, and
hospice related activities. You will also support Landmark
Physicians, Advance Practice Providers, and RN Nurse Care Managers
with important operational and clinical tasks. You will draw on
your critical thinking, clinical experience, and your positive
attitude to coordinate with and support Landmark patients and
Landmark team members.This position is full-time. Employees are
required to have flexibility to work any of our 8-hour shift
schedules during our normal business hours of 8:00am - 7:00pm. It
may be necessary, given the business need, to work occasional
overtime.--We offer 4-6 weeks of training.*All Telecommuters will
be required to adhere to UnitedHealth Group's Telecommuter
Policy.Primary Responsibilities:
- Oversee daily operations (i.e. Daily on - call schedule,
medical record uploads, management of consents, mailings, UV
schedule, IV Packet readiness)Provide regular coaching and
constructive feedback to Care Coordinators to enhance overall
effectivenessAssist in development, documentation, and
implementation of departmental policies and proceduresSkilled as a
super - user of technology (windows programs and Electronic Medical
Records (EMR's)Skilled as an educational trainer on the CC role and
function for department and marketEnsure CC documentation meets
Landmark standards and appropriate abbreviations are usedReview and
utilize Reports Dashboard data for management of the
departmentMaintains relationship with market leadership, especially
Supervisors of Clinical Operations to ensure coordination of care,
understanding of policy / practice change that impact department or
marketOther special projects or initiatives as assigned--You'll be

you may be interested in.Required Qualifications:
- High School Diploma / GED (or higher)
- 2+ years experience analyzing and solving customer
problems
- 1+ years of experience working in the health care industry
- 1+ years of experience working with medical terminology
- Ability to work any of our 8-hour shift schedules during our
normal business hours of 8:00am - 7:00pm. It may be necessary,
given the business need, to work occasional overtime.--Preferred
Qualifications:
- Experience working with health care insurance
- Experience working with Medicare and/or Medicaid Services
- Experience in a hospital, physician's office or medical clinic
setting
- Clerical or administrative support background or experience
working in a call center environmentTelecommuting

York / Rhode Island residents is $18.80 - $36.78.Pay is based on
several factors including but not limited to education, work
